This policy explains how 3twoApexSolutionsHLT&WELL (“we,” “us”) collects and uses information through this website.
Information we collect
- Assessment answers. Your name, email, optional company, job type, and your answers to the health screening, body discomfort, workstation/job task, stress and lifestyle questions, plus the scores calculated from them.
- Consultation requests (optional employer form). Company, name, work email, headcount, industry and your message.
- Chat questions. The ErgoMind assistant runs entirely in your browser; questions you type are not sent to us or stored.
- Technical data. Our hosting provider may log standard request data (such as IP address and browser type) for security and operations.
How your assessment answers are stored
While you complete the assessment, your answers are saved in your own browser’s local storage so you don’t lose progress. You can clear them at any time with the “Start over” button or by clearing your browser data. When you submit, your answers and scores are transmitted to us through our website host’s form-handling service (Netlify Forms) so we can review and follow up.
How we use information
- To generate and follow up on your assessment report.
- To respond to consultation requests and provide services.
- To improve our services. We do not sell your personal information.
Health information
The assessment asks health-related questions. It is a screening and education tool, not medical care.
Sharing
We share information only with service providers who help us run the website and our services (for example, hosting and form processing, and payment processing by Stripe for purchases), when required by law, or with your consent.
Payments are processed by Stripe. Card details are entered on Stripe’s secure checkout and never touch our servers.
Retention & your choices
We keep submissions only as long as needed for the purposes above. To request access to or deletion of your information, email support@3twoapex.com.
