How long does the assessment take?
About 10–15 minutes. Your scored report and protocol appear on screen immediately after you submit, and you can print it or save it as a PDF.
Is this a medical diagnosis?
No. The assessment is a screening and education tool. It uses established questionnaires to identify risk factors and suggest general exercises and workstation changes. It does not diagnose or treat any condition and doesn’t replace care from a physician or other licensed provider.
What is the assessment based on?
Recognized, published instruments: the PAR-Q+ general health questions, the Nordic Musculoskeletal Questionnaire (body discomfort), self-reported risk factors drawn from ROSA, RULA and REBA (workstation and job tasks), the Perceived Stress Scale (PSS-10), and U.S. physical-activity and adult sleep guidelines. Your report shows exactly which tool and cutoff produced each result.
What if I answer “yes” to a health-screening question?
You’ll still get your full report, but it will open with a recommendation to get clearance from a physician or qualified healthcare provider before starting the movement protocol, which is what the PAR-Q+ recommends.
What happens to my answers?
While you work, answers are saved only in your own browser so a refresh doesn’t lose progress. When you submit, your answers and scores are sent to 3twoApexSolutionsHLT&WELL through our website host’s secure form service so we can follow up. See our Privacy page for details.
Do you come on-site?
Yes. We work with employers across the Las Vegas valley, including Las Vegas, Henderson and North Las Vegas: casino resorts, hotels, warehouses, hospitals and offices. Remote and hybrid teams can be assessed virtually.
How do employers get started?
Online, in four steps: pick Small Team or Enterprise (monthly or one-time) and pay through Stripe; you receive an assessment link to share with your employees; employees take the 10–15 minute assessment; you receive the group report. A consultation is optional. Larger organizations that prefer to talk first can request one.
